Qualifications

• Current license as a Practical Nurse (LPN).
• CPR certification is required.
• Experience in skilled nursing is preferred.

Responsibilities

• Provide residents with direct routine care under RN supervision.
• Develop and modify the resident Plan of Care with the RN.
• Monitor and document resident health data; report changes to the RN.
• Communicate resident needs to primary caregivers and the RN.
• Ensure compliance with BON standards and HIPAA.
