OSF HealthCare is seeking a Licensed Practical Nurse Care Guide to support outpatient care coordination in Pekin, IL. The role includes registering patients, collecting payments, rooming, assisting providers, and discharge planning while ensuring a smooth patient experience.

Floating to facilities may occur based on needs. The position requires an LPN license and BLS within 2 weeks of hire, strong communication, and proficiency with Microsoft Office; Epic familiarity is preferred.

Qualifications

• High School Diploma or GED required.
• LPN license and BLS required within 2 weeks of hire.
• Excellent interpersonal and communication skills.
• Proficient with Microsoft software.
• Knowledge of Epic EHR system.
• 1 year clinical service experience preferred.

Responsabilités

• Register and aid patients through the visit workflow (registration, payments, rooming).
• Assist provider with care and treatment and discharge planning.
• Follow-up with patients after visits.
• Serve as patient advocate and coordinate with families and staff.
