Job summary

HIA Health seeks a Care Coordinator (MA/CNA) in Fargo, ND, to support Primary and Palliative Care programs with ongoing care coordination for medically complex, chronically ill, and high-risk patients. You will help with chart preparation, outreach, medication and care plan support, and coordination across care teams to promote continuity of care.

Qualifications

• High school diploma or equivalent.
• CNA or MA certification.
• 2+ years of healthcare experience (preferred).
• Familiarity with electronic medical records (EMRs).
• Strong clinical skills and interpersonal communication.
• Current certification in North Dakota and Minnesota.
• Valid driver's license and auto insurance.

Responsibilities

• Coordinate care across Primary and Palliative Care teams and external partners.
• Prepare patient charts and reconcile information from facilities and records.
• Perform patient outreach, follow-up, and education to promote continuity of care.
• Support medication lists and care plan updates within policy.
• Enter and track orders, referrals, and services; assist with prior authorizations.
• Assist with transitions of care after ED visits or hospital stays and ensure proper follow-up.
• Document CCM activities accurately in the EMR in a timely manner.
