Job summary
Monarch Healthcare Management is seeking an LPN Care Coordinator to support nursing leadership, assist with care planning, data collection, and regulatory compliance within Minnesota facilities. You will monitor resident conditions, participate in care conferences, and help ensure safe, high-quality nursing care under RN supervision.
The role requires an active Minnesota LPN license, CPR certification, and the ability to work collaboratively in a fast-paced, diverse setting while supporting
Qualifications
- Must hold a current LPN license in Minnesota and CPR/emergency care certification.
- Must meet employment eligibility per MN Department of Human Services background check.
- Excellent communication skills in verbal and written form.
- Ability to multitask and work in a fast-paced environment.
Responsibilities
- Assist with care planning, resource evaluation, and nursing process implementation.
- Monitor resident conditions and report changes to RNs.
- Support and direct LPNs and nursing assistants; assist with staffing needs.
- Ensure accurate documentation and adherence to nursing policies and regulatory requirements.
