Qualifications

Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment protocols.
• Participate in morning meetings to coordinate team communication.
• Monitor and adjust resident statuses based on changing needs.
• Complete required documentation accurately and timely.
• Coordinate admissions, discharges, and referrals.
• Maintain professional interactions with residents, families, and staff.
• Ensure compliance with Medicare, Medicaid and other programs.
