Job summary

Charleston in is seeking an LPN Supervisor (Day/NOC Shift) in healthcare to support residents with plans of care and timely treatment coordination. You will participate in daily rounds, answer questions, and ensure accurate documentation.

The role emphasizes independent work, strong communication with families and care teams, and adherence to regulatory requirements while handling admissions, discharges, and referrals.

Qualifications

• Bachelor's Degree in Nursing or Social Work required/preferred.
• RN license preferred; LVN/LPN acceptable in some settings.
• Two years clinical nursing experience preferred.
• Familiar with Medicare/Medicaid concepts and PCC knowledge.

Responsibilities

• Communicate plans of care with residents and families.
• Attend morning meetings to coordinate care.
• Answer questions about care, illness, and treatment.
• Monitor resident status and adjust plans as needed.
• Organize daily work by priorities for residents.
• Complete required documentation and charting.
• Assist with admissions and discharges.
• Coordinate referrals with providers, facilities, and payers.
• Maintain professional interactions with residents, families, staff and partners.
