Responsibilities

• Communicate regularly with residents and families about Plans of Care and treatment plans.
• Attend morning stand-ups to facilitate team communication and coordinate care.
• Monitor and adjust residents' statuses based on changing needs.
• Complete documentation timely and accurately.
