Job summary

Community Health and Wellness of Greater Torrington, Inc. is seeking a Behavioral Health Integration Specialist (LPN) to support psychiatry clinicians and the Behavioral Health team.

You will perform nursing duties, coordinate care, manage referrals, and engage patients within integrated primary and behavioral health services. Work includes preparing patients for visits, documenting vital signs, reconciling medications, and helping implement behavioral health workflows.

Qualifications

• LPN graduation from an accredited practical nursing program.
• Current Connecticut LPN license and CPR/BLS certification.
• English proficiency; Spanish preferred but not required.

Responsibilities

• Provides nursing support to psychiatry clinicians and behavioral health team.
• Obtains and documents vital signs and conducts nursing assessments.
• Completes medication reconciliation and supports medication monitoring.
• Coordinates referrals and follow-up with multidisciplinary teams.
• Maintains timely clinical documentation in the EHR.
• Supports patient engagement and education within scope of practice.
