Job summary

Community Health Alliance is seeking an experienced LPN to provide care in an outpatient behavioral health clinic across Hamilton, Middletown, and Franklin. The role reports to a Registered Nurse, NP, or Physician and supports 24/7 medically monitored care during detox processes with a trauma-informed approach.

The LPN will administer medications, monitor vitals, educate patients, and document in the EHR, while coordinating with the care team to ensure safety and continuity of care.

Qualifications

• LPN license required and active in Ohio.
• Two years of related nursing experience preferred.
• BLS/CPR certification or willingness to obtain within 30 days.
• Valid driver's license and reliable transportation.

Responsibilities

• Administers medications as directed by medical staff and records accurately.
• Monitors patient health, collects vitals, and assists with appointment flow.
• Educates patients on symptoms, medications, and self-care.
• Collects specimens for lab testing and assists with injections/vaccinations as ordered.
• Documents patient information in the electronic health record promptly and accurately.
• Coordinates with clinicians and other facilities to obtain medical records and care plans.
