Community Mental Health Affiliates, Inc. (CMHA) is seeking a full-time Licensed Practical Nurse for the IMPACT program at 233 Main St, New Britain, CT. The LPN will provide nurse care management, partner with clinicians, and coordinate health services for a caseload of clients.

Responsibilities include wellness education, health promotion, and EMR documentation. The role requires an Associate’s degree in Nursing and CT LPN licensure, with 3–5 years of nursing experience in primary care and

• Associate’s degree in Nursing (or equivalent)
• 3–5 years nursing experience in primary care with behavioral health exposure
• Knowledge of psychiatric illness and substance abuse
• Experience with EMR systems and basic MS Office tools

Responsabilidades

• Conducts home visits to assess clients and coordinate primary and behavioral health treatment
• Performs medical/health/wellness assessments and helps develop treatment plans
• Coordinates care with clinicians, transportation, and case managers to meet health goals
• Provides health education, wellness promotion, and preventive care initiatives
• Maintains documentation in EMR and ensures timely follow-up on referrals
