Job summary

Community Mental Health Affiliates, Inc. (CMHA) is seeking a full-time Licensed Practical Nurse for the IMPACT program at 233 Main St, New Britain, CT. The schedule is Monday to Friday, 8:30 am – 4:30 pm.

You will perform nurse care management services and collaborate with behavioral health clinicians to assess health needs and set care goals. You will develop wellness initiatives, document in the EMR, coordinate with primary and specialty providers, and support transportation and care

Qualifications

• Associate’s degree in Nursing or equivalent required.
• LPN license in Connecticut required.
• 3–5 years of nursing experience in primary care with behavioral health exposure.
• Experience with EMR systems and Microsoft Office.

Responsibilities

• Conducts home visits and engages clients in coordinated care.
• Performs medical/health/wellness assessments and develops goals.
• Collaborates with clinicians to implement treatment plans.
• Provides health education and wellness initiatives.
• Coordinates care with primary and specialty providers.
• Offers 24-hour on-call coverage when needed.
