Job summary

Community Mental Health Affiliates, Inc. (CMHA) is hiring a full-time Licensed Practical Nurse (LPN) for the IMPACT program at 233 Main St, New Britain, CT. Workdays are Monday–Friday, 8:30 am–4:30 pm, with a starting wage of $27.00 per hour.

The LPN will provide nurse care management, coordinate with clinicians, and contribute to wellness initiatives. CMHA emphasizes career growth and competitive benefits as part of its compensation package.

Qualifications

• Associate’s degree in Nursing (or equivalent) required.
• LPN licensure in the State of Connecticut required.
• 3–5 years of nursing experience, preferably in primary care and behavioral health.
• Experience with EMR and Microsoft Office products (Word/Outlook).

Responsibilities

• Provides nurse care management to a caseload of clients.
• Performs medical/health/wellness assessments and develops health goals.
• Collaborates with behavioral health clinicians to develop treatment plans.
• Delivers wellness, health promotion and education initiatives.
• Documents care and health data in EMR and tracks progress.
• Provides 24-hour on-call coverage as needed.
