The role requires conducting LOC assessments for the MyCare Ohio Waiver, completing re-determinations and Significant Change in Condition assessments, and traveling to consumer locations across four Ohio counties.

Qualifications

  • Licensed Practical Nurse (LPN) or BA/BS with at least 1 year in long-term care information/referral.
  • Knowledge of information and referral services for long-term care options.
  • OSHIIP certification must be obtained within one year of hire.
  • Must have a valid driver’s license and proof of auto insurance.
  • Must pass a criminal background check and drug screen.

Responsibilities

  • Assess and coordinate initial PCCP and PCSP within cost caps.
  • Provide referrals to community resources as needed.
  • Conduct LOC assessments and re-determinations for MyCare Ohio Waiver.
  • Travel to consumer locations within four-county service area (Ashtabula, Mahoning, Trumbull, Columbiana).
  • Document findings accurately and maintain required records.

Summary of Duties and Responsibilities:Under the general supervision of the Director of Home and Community Based Services, and direct supervision of the HCBS Supervisor, the Assessor is responsible for assessing and coordinating an initial person-centered care plan (PCCP) and person-centered service plan (PCSP) within designated cost caps and make referrals to available community resources. Assessors assure that the initial services for any assessed program are appropriate and meet the needs of the client to live safely in a Home and Community Based (HCBS) setting.For the NextGen MyCare Ohio Waiver program Assessors will conduct Initial Level of Care (LOC) assessments as well as LOC re-determinations and Significant Change in Condition Assessments for all consumers as requested through the Managed Care Organization Plans (MCOPs). The Assessor will utilize the assessment tool specific to MyCare, to provide a limited, detailed assessment to make an LOC determination.Must have the following abilities to perform essential functions:Ability to effectively communicate with the public, in person, by phone, and via email.Ability to comprehend written material related to essential job functions.Ability to filter Excel reports, interpret data, and identify trends.Ability to accurately document facts related to essential job functions.Ability to comprehend, recall, and apply facts related to essential job functions.Ability to analyze, evaluate, and implement a reasonable course of action based on available information.Ability to lift/carry 20 lbs. upstairs.Ability to climb steps and access private homes/locations which may not be readily accessible.Ability to travel to community and consumer locations within our four-county service area (Ashtabula, Mahoning, Trumbull, and Columbiana counties).The ability to occasionally travel outside of our four-county service area including but not limited to Columbus, Ohio.Minimum Job Qualifications:The Applicant is preferred to be a Licensed Practical Nurse, or Social Work Trainee or have completed a Bachelor of Arts degree or a Bachelor of science degree and also possess at least one year of experience in one or both of the following:Providing individuals with information about options available to meet long-term care needs or providing individuals with related information such as that which is provided through information and referral, information and assistance.Managed care experience or other such healthcare experience related to individuals' long-term care needs.The applicant will require oversight from an appropriate assigned licensed staff member.Applicant shall possess the knowledge and skills necessary to provide the level of assessment required by the client and by any legal regulations.OSHIIP certification must be obtained within one year of hire date.Applicant must possess a valid driver’s license and proof of automobile insurance.Must pass a criminal background check and drug screen.
