Job summary

PruittHealth is seeking an LPN Assessment Nurse to perform initial and ongoing home-based assessments for elderly and disabled waiver program members. The role involves evaluating IADLs/ADLs, documenting in Harmony, and coordinating with the Medical Director and Home First team.

The successful candidate will review medications, ensure appropriate services, and participate in mandatory in-services while complying with all policy guidelines and regulations.

Qualifications

• Must be computer literate and able to learn new programs.
• Must communicate with translators for non-English-speaking clients.
• Maintains client confidentiality and demonstrates integrity at all times.

Responsibilities

• Conducts initial assessments and reassessments in clients' homes for the elderly and disabled waiver program.
• Reviews medications for appropriateness and notes changes.
• Documents assessments in Harmony system and requests medical records as needed to determine eligibility.
• Participates in team meetings with Medical Director and Home First team.
• Assists with implementation of waiver program guidelines and client goals.
