Job summary

PruittHealth in Georgia-Blue Ridge is seeking a Licensed Practical Nurse to screen referrals, coordinate admissions, and liaise with providers and home health teams.

The role requires a state-approved nursing education, at least five years of nursing experience (hospice or SOURCE care preferred), a valid driver license, and willingness to participate in on-call rotations and trainings.

Qualifications

• Must have a valid LPN license.
• Graduate of a state approved accredited nursing school.
• Minimum of five (5) years of nursing experience; Hospice or SOURCE care preferred.

Responsibilities

• Screen referrals and coordinate admission processes.
• Coordinate services with providers and billing reconciliation.
• Engage with community referral sources for awareness and referrals.
• Complete new admission paperwork and home assessments.
• Attend weekly team meetings with Medical Director and present new admits and discharges.
• Complete quarterly home and PCP visits with required documentation.
• Track hospital/ER visits and manage records and communications.
• Arrange transportation and travel using the driver license; participate in audits and trainings.
