Job summary

Berkshire County Arc, Inc. in Massachusetts is seeking a Licensed Practical Nurse (LPN) to support individuals with ID/DD populations or the elderly. You will perform clinical assessments, develop and review AFC care plans, and oversee AFC caregivers in collaboration with the care manager.

The position is hourly, full-time, with a Mon-Fri schedule that totals 40 hours per week. A $3,500 sign-on bonus is offered, and MA licensure plus a valid driver’s license are required.

Qualifications

• Licensed Practical Nurse with two years experience with ID/DD populations or the elderly.
• Current license in the State of Massachusetts.
• Valid driver’s license and insured vehicle.

Responsibilities

• Complete and coordinate all applicable clinical assessments and clinical evaluations.
• Develop and review on an ongoing basis each member’s AFC plan of care.
• Provide oversight to AFC caregivers in conjunction with the care manager.
• Review the PCP Summary Form.
• Monitor the health status of all members to ensure that all needed AFC is properly delivered.
• Report changes in the health status of any member to the member’s PCP.
• Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6).
