Northeast Arc in Massachusetts seeks Per Diem LPNs to provide direct care in residential settings, following care plans, monitoring health, and reporting changes to the on-call RN. You will collaborate with residential staff to support individuals in community residences and participate in health-related trainings for staff.

• Massachusetts LPN license required.
• Experience with intellectual/developmentally disabled individuals is a plus.
• Valid Driver’s license and good driving record required.

Responsabilidades

• Provide direct care to individuals in a residential setting following the plan of care.
• Monitor medical status and report changes to the on-call RN Case Manager.
• Work with residential staff to support individuals in the community residence.
• Assess individuals during shifts and address nursing needs; document appropriately.
• Assist at physician appointments, community trips, and other activities as needed.
• Prepare health-related in-service for staff and supervise health-related routines.
