Job summary

Gurwin Healthcare System is seeking a full-time Clinical Assessor / Coordinator, LPN in Commack, NY. The role focuses on reviewing sub-acute and long-term care referrals and determining appropriate placement within Gurwin Jewish Health Care System.

Required: New York LPN license and at least 1 year of hospital, long-term care, or CHHA experience. Monday–Friday, 9:00am–5:00pm with a comprehensive benefits package and sign-on bonus.

Qualifications

• Current NY State LPN license.
• Minimum of 1 year experience in hospital, long-term care, or CHHA.
• Experience reviewing sub-acute referrals and patient disposition.

Responsibilities

• Interpret PRI/RUG classification system.
• Manage referral caseload and respond via Navi/Allscripts and eFax.
• Accurately assess the classification and patient disposition on screening.
• Coordinate referrals using EMR including verification of benefits.
• Coordinate bed board/census when needed.
• Assess potential patients at partnering hospitals.
