Job summary

Gurwin Healthcare System in Commack, NY seeks a part-time Licensed Practical Nurse for all shifts (7:00am-3:15pm, 3:00pm-11:15pm, 11:00pm-7:15am, every other weekend). Graduation from an accredited program and NYS LPN license are required to care for geriatric patients under RN supervision.

We offer a $10,000 sign-on for full-time and $5,000 for part-time hires, plus a rate increase to $36.19–$39.81 after six months, plus a robust benefits package and opportunities for professional growth.

Qualifications

• Graduate of an accredited nursing program with NYS license.
• Interest in geriatric patient needs and principles of geriatric nursing.
• Ability to establish effective interpersonal relations with patients, staff and families.
• Ability to communicate effectively in English, both orally and in writing.
• Education in LTC/Subacute to ensure competency on all units.

Responsibilities

• Participates in nursing care of patients within LPN scope under RN supervision.
• Contributes to formulation and implementation of patient care plans for geriatric patients.
• Administers patient care as assigned by an RN within guidelines; may lead team or act as charge nurse.
• Directs and supervises non-professional staff as assigned.
• Observes, records, and reports patient data per policy.
• Participates in unit nursing and inter-disciplinary team activities.
• Administers medications and nursing procedures per policy.
• Attends in-service programs and participates in committees as assigned.
• Performs related duties; may assist in emergencies.

Gurwin Healthcare System is a full healthcare continuum providing extraordinary care to the Long Island community and beyond. Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin's Assisted Living community (Gurwin Jewish : Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies.

POSITION

Requirements include

• Graduate of an accredited professional nursing program with a current license to practice in New York State.
• Demonstrates interest in the special needs of geriatric patients and possesses an ability to integrate principles of geriatric nursing into nursing practice.
• Ability to establish effective interpersonal relations with patients, staff and families.
• Ability to communicate effectively in English, both orally and in writing.
• Education in all areas of LTC/Subacute to ensure competency on all units in the facility.

Responsibilities include, but are not limited to

• Participates in the nursing care of patients within the scope of the legal definition of the practice of practical nursing. The LPN works under the supervision and/or direction of a registered professional nurse.
• Participates in the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients under the supervision of an RN.
• Administers patient care as assigned by an RN within departmental guidelines. As the unit situation requires, an LPN may be assigned to assume team leader, primary nurse or charge responsibilities under the direction of an RN
• Directs and supervises non-professional staff as assigned.
• Observes, records and reports patient data as dictated by departmental policy.
• Participates in unit nursing and inter-disciplinary team activities.
• Administers medications and performs nursing procedures as dictated by departmental policy.
• Attends nursing and other institutional in-service programs and participates in departmental committees as assigned.
• Performs related and miscellaneous duties as directed. In an emergency situation, may be expected to perform unrelated duties.
