Job summary

Gurwin Healthcare System seeks a full-time Clinical Assessor / Coordinator, LPN. You will review referrals from hospitals and facilities, assess patient needs, and determine proper placement within Gurwin’s care continuum.

The role requires NY LPN licensure and at least one year of hospital, LTC, or CHHA experience. Strong EMR and communication skills are essential for coordinating care and admissions.

Qualifications

• Licensed to practice as LPN in New York State.
• Minimum of one year experience in hospital, long term care or CHHA.

Responsibilities

• Review sub-acute and long-term care referrals from area hospitals.
• Identify health problems and determine appropriate classification and care regimes.
• Coordinate referral via EMR and verify benefits.
• Coordinate bed board/census when needed.
• Communicate with health care team and families clearly and professionally.
