Job summary

Woburn Rehabilitation and Nursing Center is hiring a compassionate LPN to provide direct nursing care, supervise CNAs, and collaborate with an interdisciplinary team to ensure high-quality, resident-centered care in a 140-bed facility in Woburn, MA.

The role includes administering medications, wound and IV care, and creating/Updating care plans, with a $5,000 sign-on bonus for eligible employees and weekly pay. A supportive work environment and ongoing education are offered.

Qualifications

• Current, valid, unencumbered Massachusetts Licensed Practical Nurse (LPN) license.
• Strong clinical judgment, communication, and critical-thinking skills.
• Ability to handle challenging situations with professionalism, tact, and compassion.
• Ability to work effectively with residents, families, and interdisciplinary healthcare professionals.
• Previous long-term care or skilled nursing experience preferred, but not required.
• Dependable, compassionate, and committed to providing safe, high-quality resident care.

Responsibilities

• Coordinate and supervise the daily activities of CNAs and other nursing personnel on assigned units.
• Promote compliance with facility policies, procedures, professional standards, and applicable regulations.
• Participate in resident admissions, discharges, and transfers.
• Monitor residents for changes in condition and report concerns to appropriate healthcare professionals.
• Transcribe and carry out physician orders per procedures.
• Administer medications as prescribed and monitor responses and adverse effects.
• Provide wound care, colostomy care, tube feedings, suctioning, IV administration within scope.
• Conduct regular rounds to assess resident needs and ensure staff follow through.
• Collaborate with the interdisciplinary care team to provide comprehensive, individualized resident care.
• Initiate, review, and update resident care plans as needed.
• Provide compassionate end-of-life care as required.
• Monitor medication, supply, and equipment levels and notify when resources are needed.
• Promote teamwork and effective communication among staff.
• Participate in regulatory surveys, inspections, and QA activities.
• Protect resident dignity, rights, privacy, and confidentiality.
