Job summary

Stellar Health Group is seeking a dedicated Registered Nurse (RN) at Woburn Rehabilitation and Nursing Center to provide high-quality, resident-centered care. You will work with an interdisciplinary team to develop individualized care plans and promote positive clinical outcomes.

Eligible candidates hold an active Massachusetts RN license and demonstrate strong assessment, communication, and teamwork skills.

Qualifications

• Current, active, and unrestricted Massachusetts RN license.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and interpersonal skills.
• Ability to collaborate with residents, families, and interdisciplinary team members.

Responsibilities

• Provide direct nursing care to residents, including administering medications and monitoring conditions.
• Develop and implement individualized resident care plans with the interdisciplinary team.
• Observe residents and report changes in condition promptly.
• Document care and observations accurately for continuity of care.
• Participate in quality improvement initiatives and uphold standards of excellence.
