Job summary

Oak Hills Care Center is seeking a Licensed Practical Nurse (LPN) for full-time days to provide direct nursing care and supervise CNAs and CMAs in a skilled nursing facility in Oklahoma.

You will follow the Oklahoma Nurse Practice Act and uphold safety, accurate charting, medication administration, and effective communication with residents and families. The role requires compassion, reliability, and strong teamwork in a caregiving environment.

Qualifications

• Must be a Licensed Practical Nurse (LPN) in good standing and currently licensed by the state of Oklahoma.
• Minimum 1 year experience in a Skilled Nursing Facility or Long-Term Care environment is required.
• Diploma from an accredited School of Nursing is required.
• Ability to read, write, and follow oral and written directions at a level necessary to accomplish the job is required.
• Must have excellent communication and teaching skills in both individual and group settings.

Responsibilities

• Provide direct nursing care to residents and supervise day-to-day nursing activities performed by CNAs and CMAs.
• Record and transcribe physician orders to residents' medical records.
• Take and record vital signs and perform personal care tasks as needed.
• Administer medications following the 5 Rights of Medication Administration.
• Maintain infection control and ensure charting is complete and accurate.
• Communicate with residents, families, and staff; participate in QA and surveys.
• Assist with discharge planning and care plan updates.
