Job summary

Great Lakes Management is seeking an LPN for a 20-hour-per-week role at The Preserve in Roseville, MN. The position focuses on delivering quality assisted living care to residents, adhering to state regulations and care plans.

The ideal candidate holds an active MN LPN license, with at least one year in home health, assisted living, or long-term care, and exhibits strong organizational and teamwork skills to support our residents and care team.

Qualifications

• Current state license as a LPN per state regulations.
• Minimum of one year's experience in home health, assisted living or long-term care preferred.
• Good organizational skills with initiative and minimal supervision.
• Exhibits a caring heart with a passion to care for the elderly.

Responsibilities

• Ensures residents meet admission criteria and coordinates placement if necessary.
• Ensures residents have up-to-date care plans per state regulations.
• Monitors and maintains resident charts and clinical progress notes.
• Communicates changes in residents' needs or conditions to physicians, staff and family.
• Facilitates coordination of resident services with other providers.
• Oversees medications and renewals and follows up with physicians as needed.
• Ensures medication administration procedures are followed and discusses corrections with staff.
• Documents information in the resident log for staff awareness.
• Provides alternating supervisory visits in coordination with the RN.
• Works as a team with all members of the residents’ care team.
• Supervises medication practices and medical equipment in use.
• Ensures staff understand and follow all care procedures.
• Shares resident and family concerns with pertinent staff when appropriate.
• Works with the manager to ensure emergency procedures are in place.

LPN R esponsibilities:

• Assures residents maintain admission criteria, monitor , and recommend alternative placement if necessary.
• Assures residents have up to date care plans per state regulations

• Communicates to physicians, staff and family changes in resident's needs or conditions.
• Facilitates coordination of resident services with other service providers.
• Oversees medications and renewals and any necessary follow-up with physicians about medications and/or medical conditions.
• Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
• Communicates in resident's log any information that staff need to be aware of.
• Provides alternating supervisory visits in coordination with the RN .
• Works as a team player with all members of the residents’ care team.

• Assures that staff clearly understand and follow all resident care procedures.
