Job summary

Maxim Healthcare Services, Inc. in Cleveland is hiring a Licensed Practical Nurse (LPN) or a Registered Nurse (RN) to provide in-home care for pediatric and/or adult medically fragile patients.

The role requires delivering skilled nursing care using the nursing process, evaluating patient needs, and coordinating care with families and physicians in a home setting.

Qualifications

• Licensed as an LPN/LVN in the state where practice is required.
• Current TB test or Chest X-Ray.
• Current BLS certification.
• One year of professional nursing experience preferred.

Responsibilities

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide skilled nursing services per prescribed orders in home settings.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and document data and nursing diagnoses.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.


• On each assignment, provides skilled nursing care/services in accordance with prescribed orders


• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.


• Collects, analyzes, and interprets data and information from health care members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.


• Current TB or Chest X-Ray.


• Current BLS card.


• One year prior professional nursing experience preferred
