Job summary

Silver Stream Rehabilitation and Nursing Center is seeking compassionate LPNs to join our nursing team for per-diem, all shifts. The role involves administering medications, monitoring residents, and providing direct care in line with facility policies.

Strong clinical and communication skills are essential. Experience in long-term care is preferred but not required; teamwork and resident-centered care are emphasized in this opening.

Qualifications

• Current LPN license in the Commonwealth of Pennsylvania.
• Long-term care experience preferred, but not required.

Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team.

Responsibilities:

• Administer medications and treatments as prescribed

• Monitor residents' conditions and report changes to the RN Supervisor and physicians

• Provide direct nursing care in accordance with facility policies and standards of practice

• Accurately document resident care and services provided

• Supervise and support Certified Nursing Assistants (CNAs)

• Participate in care planning and interdisciplinary team meetings

• Ensure compliance with all state, federal, and facility regulations
