Job summary

Southbridge Rehabilitation & Health Care Center is seeking per diem Licensed Practical Nurses (LPNs) for all shifts to deliver direct nursing care under RN supervision and to supervise day-to-day nursing activities for residents.

Weekly pay ranges from $35 to $40 per hour, with shift differentials for evenings, overnights, and weekends. New graduates are welcome, and eligibility for CPR certification is required to join the team.

Qualifications

• Must be a graduate of an approved LPN/LVN program.
• Must CPR certified.
• Must possess a current, unencumbered license to practice as an LPN/LVN in this state.

Responsibilities

• Meet the nursing and rehabilitation needs of each resident.
• Maintain current resident care plans on all residents under his/her care.
• Transcribe physician’s orders promptly and accurately.
• Review patient records daily to assure accuracy and completeness.
• Document comprehensive and complete nursing notes.
• Visit each resident daily to evaluate resident needs.
• Maintain accurate awareness of residents on alert list.
• Document and report any unusual or significant findings; contact the physician when necessary.
• Administer medication according to procedure.
• Maintain an accurate and safe control of all drugs for which he/she is responsible.

Southbridge Rehabilitation & Health Care Center is looking for per diem LPNs on all shifts!

Weekly pay ranges from $35 to $40 an hour, depending on the shift. Evenings, overnights, and weekends have shift differentials.

As a Licensed Practical Nurse, your job position is to provide direct nursing care under the supervision of the RN to residents and to supervise the day-to-day nursing activities performed by the nursing assistant to ensure that the highest degree of quality care is maintained at all times.

Experience & Education:

• Must be a graduate of an approved LPN/LVN program.
• Must be CPR certified.
• Must possess a current, unencumbered license to practice as an LPN/LVN in this state.
• New graduates welcome!
