Clarendale West End, located in Nashville, Tennessee, is seeking an LPN to deliver direct nursing care to residents and supervise Quality of Life Specialists/Resident Assistants. This full-time opportunity offers a 7:00am–7:00pm shift with every other weekend required and a competitive pay rate of $33–$34 per hour.

You will assist residents with activities of daily living, monitor health changes, contribute to care plans, and coordinate with other disciplines under physician orders.

Qualifications

• LPN is required.
• Current nursing license valid in state worked.
• Willingness to be available for emergencies.

Responsabilités

• Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent life-style of residents.
• Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
• Provide daily report to the Director of Health Services concerning community conditions, personnel performance, resident conditions, and needs for nursing supplies.
• Provide nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.
• Ensure preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.
