Job summary

JourneyCare Hospice in Lake Forest, IL is seeking a Hospice LPN to join our growing team. This field-based role follows the plan of care, delivers treatments, and collaborates to support patients and families.

Hours are Monday-Friday 8:30-5, with no on-call or weekends. Salary ranges from $26–$38 hourly, based on experience, and includes comprehensive benefits and mileage reimbursement.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical preferred.
• Currently licensed as an LPN/LVN in the practicing state and CPR certified.
• Strong verbal and written communication and good organizational skills.
• Valid driver's license with insured vehicle; understanding hospice needs.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act.
• Implement care per a comprehensive assessment and plan of care.
• Document patient services accurately and timely to reflect the plan of care.
• Educate patient and family/caregiver on diagnosis and self-care.
• Coordinate hospice services and report needs to the interdisciplinary team.
• Manage pain and symptoms and evaluate treatment responses.

JourneyCare Hospice is seeking a Hospice LPN to join our growing team in the Lake Forest, IL area! This is a field-based position responsible for following the plan of care for our patients, providing treatments, and working collaboratively to help our patients and their families.

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver's license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the terminally ill

What You'll Do:

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications
