JourneyCare Hospice in Lake Forest, IL is seeking a Hospice LPN to join our growing team in the Lake Forest area. This field-based role follows the plan of care for patients, provides treatments, and collaborates to support patients and families.

Hours are Monday–Friday, 8:30–5:00 with no on-call or weekends. Comprehensive benefits and a culture of care accompany this opportunity.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Demonstrates good verbal and written communication, and organization skills.
• Valid driver’s license with an automobile that is insured per state requirements.
• Understands hospice philosophy and the needs of the terminally ill.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participate in the coordination of hospice services, reporting identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management and evaluate patient response to treatments/medications.
