Job summary

Miracle City Hospice is hiring an LPN Continuous Care Nurse to support Broadview Heights and surrounding counties. Join a compassionate team of clinicians delivering end-of-life care with a focus on patient comfort and family support.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice/medical-surgical preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Good verbal and written communication and organizational skills.
• Valid driver’s license with insured vehicle as per state requirements.
• Understands hospice philosophy and needs of terminally ill.

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act.
• Implement the plan of care based on comprehensive assessment.
• Provide accurate and timely documentation of patient services.
• Educate patients and families about diagnosis and self-care.
• Coordinate hospice services and report needs to the interdisciplinary group.
• Manage pain and symptoms and evaluate response to treatments.

What You�ll Do:

• Provide direct patient care as defined in State Nurse Practice Act
• Implement current nursing practice following a comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services to reflect the plan of care
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications

Qualifications:

• Graduate of an accredited practical nurse or a vocational nursing program
• Three years of nursing experience. Community health, hospice or medical/surgical experience is preferred
• Currently licensed as an LPN/LVN in the practicing state
• Must possess current CPR Certification
• Demonstrates good verbal and written communication, and organization skills
• Valid driver’s license with an automobile that is insured in accordance with state requirements
• Understands the philosophy of hospice concept and the needs of the termin ally ill
