Job summary

JourneyCare Hospice is seeking a Hospice LPN to join our Crisis Care team in Crystal Lake, IL. This field-based role follows the plan of care for continuous care patients, provides treatments, and collaborates to support patients and families.

Hours are Monday – Friday with three 12-hour shifts 7AM–7PM; on-call every other weekend. Salary ranges from $26 to $38 hourly, based on experience.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience; hospice or medical/surgical experience preferred.
• Currently licensed as an LPN/LVN in the practicing state.
• Must possess current CPR Certification.
• Valid driver’s license with an automobile that is insured in accordance with state requirements.

Responsibilities

• Provide direct patient care as defined in State Nurse Practice Act.
• Implement current nursing practice following a comprehensive assessment and the plan of care.
• Provide accurate and timely documentation of patient services to reflect the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and self-plan of care.
• Participate in the coordination of hospice services, appropriately reporting the identified needs to the interdisciplinary group.
• Provide appropriate pain/symptom management. Evaluates the patient’s response to treatments/medications.
