Storybrook Care and Rehabilitation in Fort Collins, CO is seeking compassionate LPNs/LVNs to deliver high‑quality care in a skilled nursing setting. Full‑time and PRN shifts available, with a sign‑on bonus for full time or part time applicants.
Shifts include 6PM–6AM, 6PM or 2AM; 12‑hour and 8‑hour options. Competitive pay range from $28 to $40 per hour, plus benefits and earned wage access. Relevant licensure required and a commitment to resident well‑being.

This position to ensure that quality care is delivered through monitoring of staff performance, physician contact, care planning and daily resident observation. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities.
