Job summary

Jobtailor seeks a licensed practical nurse (LPN/LVN) to provide direct nursing care to residents and supervise nursing assistants in a skilled nursing facility. You will coordinate shift services, enforce policies, and work with physicians to admit, transfer, and discharge residents.

Applicants must hold an active SC LPN/LVN license, CPR, and maintain continuing education. Strong leadership, documentation, and ability to move and lift are required.

Qualifications

  • Nursing degree from an accredited college or university, or graduation from an approved LPN/LVN program.
  • Current, unencumbered, active South Carolina LPN/LVN license required.
  • Current CPR certification.
  • Must maintain required continuing education and licensing.
  • Must remain in good standing with the State Board of Nursing.
  • Must be able to read, write, speak, and understand English.
  • Ability to read technical procedures and policy and procedure manuals.
  • Knowledge of nursing and medical practices, procedures, laws, regulations, and nursing facility guidelines.
  • Leadership and supervisory ability.
  • Ability to make independent decisions when circumstances warrant.
  • Ability to plan, organize, develop, implement, and interpret nursing programs, policies, and procedures.
  • Knowledge and experience with PCC preferred.
  • Ability to move intermittently throughout the workday.
  • Ability to push, pull, move, and/or lift at least 25 pounds to a minimum height of 5 feet and carry it at least 50 feet.
  • Must complete required medical and physical examination.
  • Must be able to work with ill, disabled, elderly, emotionally upset, and sometimes hostile people.
  • May need to assist with resident evacuation during emergencies.

Responsibilities

  • Provide direct nursing care to residents
  • Supervise day-to-day nursing activities performed by nursing assistants
  • Direct nursing assistants and coordinate shift services, programs, and activities
  • Ensure compliance with facility policies, procedures, and applicable federal, state, and local regulations
  • Admit, transfer, and discharge residents
  • Complete medical forms, reports, evaluations, studies, charting, and incident documentation
  • Receive and transcribe physician orders and maintain resident medical records
  • Prepare and administer physician-ordered medications and treatments
  • Monitor medication supplies, narcotic records, and medication discrepancies
  • Participate in staffing, performance evaluations, work assignments, and personnel supervision
  • Conduct daily rounds and provide leadership to nursing personnel
  • Coordinate resident care with physicians, families, social services, dietary, and other departments
  • Review and maintain resident care plans and assessments
  • Perform nursing procedures including catheterization, tube feedings, suction, wound dressing changes, colostomy and drainage bag care, specimen collection, vital signs, and range-of-motion exercises
  • Monitor residents' physical and emotional status and notify physicians and next of kin of condition changes
  • Participate in restorative, rehabilitative, infection-control, safety, sanitation, and quality-assurance programs
  • Develop and participate in staff orientation, in-service training, and continuing education
  • Maintain adequate supplies and equipment and ensure safe operation
  • Provide data to the Quality Assurance & Assessment Committee as requested
