Job summary

Jobtailor is seeking an LPN to provide direct participant care to frail elderly individuals within a healthcare team setting. You will review tests, administer medications, and maintain accurate medical records while ensuring adherence to practical nursing standards.

Responsibilities include discharge planning support, collaborative care planning, data collection, and advocacy for patient needs, with a focus on quality improvement and safe medication practices.

Qualifications

• Must have 1 year of experience working with the frail elderly population.
• Must have 1 year of experience working as an LPN.
• Graduate of an accredited LPN program.
• High school education.
• Proficient in English.
• Verbal and written communication skills.
• Computer skills.

• Review diagnostic test results and immediately report abnormal results or changes in participant condition.
• Administer prescribed medications, therapies, and treatments according to practical nursing standards.
• Review and maintain participant medical records, notes, and physician orders.
• Document all participant care activities according to documentation standards.
• Implement plan-of-care components within the LPN scope of practice and competency.
• Assist with discharge planning and participant teaching under healthcare team direction.
• Complete medication orders in PACE Care and assist with medication documentation and refills.
• Collaborate with the supervising healthcare team on participant plan-of-care execution.
• Assist physicians, RNs, and healthcare team members with data collection, assessment, planning, intervention, and evaluation.
• Advocate for participants' psychological, ethical, spiritual, and cultural needs.
• Participate in unit teams, work groups, committees, and quality improvement initiatives.
• Enter medications accurately into PACE Care to help ensure zero medication errors.
• Report medication delivery and process problems to the interdisciplinary team.
