Job summary

Total Life in California seeks a Nurse Practitioner to lead longevity days at partner senior living communities in the Lodi-Sacramento area. You will conduct comprehensive geriatric assessments, perform longevity visits, and coordinate follow-up care with residents and their primary care providers.

You will be trained by our Chief Medical Officer and work with a team to deliver education to residents and families, with on-site days on Weekdays.

Qualifications

• Active, unrestricted California RN license
• Nurse Practitioner certification from California Board of Registered Nursing
• Active NP Furnishing Number from CNBR
• Current national NP board certification
• Active DEA registration for California and CURES registration
• Experience with Medicare documentation and E/M coding
• Willingness to travel to Lodi-Sacramento area sites
• Weekday availability for on-site days in Lodi-Sacramento area

Responsibilities

• Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: assess resident priorities, review medications, deprescribing, cognition (Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function, and advance care planning
• Evaluate eligible residents for weight management, including GLP-1 therapy where appropriate
• Identify residents needing follow-up care (cognitive review, medical follow-up, psychiatry or therapy) and coordinate with primary care providers
• Document every encounter accurately to Medicare standards in our EHR
• Represent Total Life professionally with residents, families and community staff
• Travel to assigned senior living communities in the Lodi-Sacramento area
