Greenwood Center for Nursing and Rehab in Tamaqua, PA is seeking a dedicated Registered Dietitian (RD) to lead medical nutrition therapy, assess residents, and coordinate nutrition services within our skilled nursing facility.

The role emphasizes collaboration with nursing and interdisciplinary teams, education for residents and families, and ongoing evaluation of care plans to support optimal resident outcomes.

Qualifications

• Licensed RD with active credential.
• Two+ years in long-term care with admin duties, or one year as RD consultant in LTC.
• Strong communication, organizational and clinical skills.

Responsibilities

• Conduct dietary assessments for residents.
• Develop individualized dietary care plans with goals.
• Engage residents and families in care plan development.
• Ensure staff follow care plans in daily services.
• Review medical records and recommend nutrition therapy.
• Identify equipment or utensils needed in care plans.
• Collaborate with nursing and interdisciplinary teams.
