We are hiring for a Medical Social Worker.

This is a Part-Time position doing Home Visits for Hospice. Candidates must be licensed as a Social Worker in OH, with a Master's degree preferred. 20-28 hrs/week. Flexible schedule - Friday mornings required.

Responsibilities

The Medical Social Worker provides social services under the direction of a physician and Interdisciplinary Group to address social and emotional factors affecting the patient's health and develop coping mechanisms.

• Assist patients, families, physicians, and healthcare staff in understanding personal, emotional, environmental, and social factors related to health status.
• Contribute to the development of a comprehensive, integrated Plan of Care for patients daily.
• Instruct healthcare team members on community resources available to assist patients.
• Function as Bereavement Coordinator and supervise bereavement services as needed.
• Establish a bereavement care plan addressing needs up to 13 months post-patient death.

Education and Experience

License Requirements

• Master's Degree in Social Work from an accredited school.
• One year of social work experience in a healthcare setting.
• Current CPR certification.
• Valid Driver's License, vehicle insurance, and access to transportation.

Additional State-Specific Requirements

• OH: Must be licensed as a Social Worker by the State Board of Social Work.
