The founders of Day City Hospice took the lessons learned from years of experience in corporate America to create a patient-centered hospice that maximizes resources at bedside while being operated locally. When end-of-life care is necessary, patients and their families are in their most vulnerable time. Our Ohio-based management team, through our local clinicians, can respond to the needs of our patients in a timely and appropriate manner because we are local.

We are also passionate about providing our employees the right tools to do their jobs effectively and provide the best hospice care in the Dayton, OH area. We don’t require high caseloads or unmanageable quotas, which gives our team more time with each of their patients. Our top priority is our patients and the people serving them.

Schedule: Monday- Friday 8a-5p with occasional evening and weekend call

What You’ll Do

  • Perform psychosocial assessments of patients and families/caregivers in a timely manner and ensure needs are addressed in the patient’s plan of care. Provide assessments at the patient’s residence and assist when safety is an issue, with alternative plans as needed.
  • Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members to ensure consistent high-quality service.
  • Counsel patients and families/caregivers, as well as staff, regarding stress and coping difficulties.
  • Document patient services accurately and promptly to reflect the plan of care.
  • Assist with applications for SSI, Medicaid, and other resources; connect patients and families with community social, health, and welfare agencies.
  • Serve as liaison between patients, families, caregivers, and community agencies.

Qualifications

  • Social Work Degree (Bachelor’s or Master’s depending on state requirements)
  • Current State Licensure in Social Work (LSW, LMSW, LCSW, etc., as per state)
  • Minimum of one year’s experience in healthcare or hospice setting
  • Valid driver’s license and proof of insurance
