At Crossroads Hospice & Palliative Care, the goal is to provide comfort, dignity, and clarity at the end of life. The team is committed to supporting patients and families emotionally and in navigating care decisions throughout the journey.

Our Social Workers play a vital role by guiding families through complex healthcare processes, explaining disease progression, connecting them to resources, and offering grief support beyond the bedside. This role is supported by a dedicated Social Work professional who understands the depth and demands of this work.

Social Worker (LSW) Hospice Qualifications:

• Licensed Medical Social Worker
• Master’s or Bachelor’s degree from an accredited program by the Council of Social Work Education
• At least 1 year of experience in a medical setting, with hospice and/or palliative care experience
• Ability to work in both hospice & palliative programs
• Provide emotional, social, and grief support to patients and families, collaborating with spiritual and bereavement staff
• Participate in Interdisciplinary Group meetings to identify social-psychological issues related to medical problems
• Assist with coordination and participation in programs like Gift of a Day and Veteran Recognitions
• Reliable personal vehicle, valid driver’s license, and ability to operate it safely for patient visits

Schedule & Benefits:

• Schedule: Monday to Friday, 8:30 AM - 5:00 PM
