Qualifications

  • Bachelor's in Social Work required; Master's preferred.
  • Current State Licensure in Social Work (Ohio: LSW, LISW, or LPSW-S).
  • Minimum of one year’s experience in healthcare or hospice.

Schedule: Monday-Friday, 8 am - 5 pm

What You'll Do
  • Perform psychosocial assessments of patients and families/caregivers in a timely manner, ensuring identified needs are addressed in the patient's plan of care. Provide assessments at the patient's residence and assistance if the environment is unsafe, with alternative plans as needed.
  • Demonstrate exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members to ensure consistent, high-quality service.
  • Counsel patients, families, caregivers, and staff regarding stress and coping difficulties.
  • Provide accurate and timely documentation of patient services reflecting the plan of care.
  • Assist with applications for SSI, Hospice, Medicaid, and Medicaid Waivers. Obtain resources from community agencies to meet patient and family needs.
  • Serve as liaison between patients, families/caregivers, and community agencies.
Qualifications
  • Social Work Degree (Ohio: Bachelor's in Social Work; Master's preferred). Degrees in Psychology, Sociology, or related fields are acceptable for Bachelor's.
  • Current State Licensure in Social Work (Ohio: LSW, LISW, or LPSW-S).
  • Minimum of one year's experience in healthcare or hospice setting.
  • Valid driver's license and proof of insurance.
