Job summary

Horizon Home Health & Hospice in Idaho is seeking a licensed social worker to join a compassionate interdisciplinary team. You will assess psychosocial status of patients and families facing terminal illness and coordinate care in partnership with the registered nurse.

You must hold LMSW or LSW in Idaho and anticipate Oregon licensure within a reasonable time. A valid driver's license and home health/hospice experience are preferred to support comprehensive patient care.

Qualifications

• LMSW/LSW licensed in Idaho.
• Will need to be licensed in Oregon within a reasonable time frame.
• Valid Driver's License.
• Home Health and Hospice experience preferred.

Responsibilities

• Assesses psychosocial status of patients and families/caregivers related to the patient’s terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
• Carries out social evaluations and plans intervention based on evaluation findings; counsels patient and family/caregivers as needed.
• Maintains clinical records on all patients referred to social work.
• Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
• Provides information to patients and families/caregivers and community agencies.
• Serves as liaison between patients and families/caregivers and community agencies.
• Maintains collaborative relationships with organization personnel to support patient care.
• Maintains and develops contracts with public and private agencies as resources for patient and personnel.
• Maintains and assists the volunteer coordinator in establishing a need for volunteer services with the patient and family/caregiver and entire hospice team.
• Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings.
• Adhering to the organization’s documentation and care procedures and standards of personal and professional conduct.
• Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
• Actively participates in quality assessment performance improvement teams and activities.
• Other duties as delegated by the Clinical Supervisor.
