Job summary

Enhabit Home Health & Hospice in Idaho seeks an experienced Licensed Master Social Worker to join our patient-focused care teams. You will provide intermittent social work services, educate patients and families, and collaborate with physicians to implement the plan of care.

The role requires an MSW from a CSWE-accredited program, current state licensure, and at least one year of health-care social work experience; Medicare home health or hospice experience is preferred.

Qualifications

• Must have a masters degree from a school of social work accredited by the council on social work education.
• Must be certified or licensed in the state where they currently practice.
• Must have at least one year of social work experience in a health care setting.

Responsibilities

• Administer social services to patients requiring intermittent professional services.
• Teach the patient, family, and other members of the health care team.
• Perform services in accordance with the physician’s orders and the established plan of care (POC).
