Job summary

Heritage Home Health and Hospice is seeking a licensed social worker to join our team delivering in-home care across Idaho. The role involves visiting patients in their homes, coordinating care with families and health professionals, and ensuring adherence to agency and state guidelines.

The ideal candidate holds an Idaho LSW/LMSW/LCSW license, is BLS CPR certified, and is committed to high-quality, compassionate patient care in a supportive interdisciplinary environment.

Qualifications

• Be licensed in Idaho as a Licensed Social Worker (LSW), Licensed Master’s Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW).
• Be BLS CPR certified.
• Home Health and Hospice experience preferred, but not required.
• Meet all licensing requirements related to your clinical license.

Responsibilities

• Provide visits in patients' place of residency and coordinate care with the care team.
• Collaborate with interdisciplinary team and community agencies to connect patient care.
