Job summary

UAB Medicine in Birmingham, AL is seeking a licensed social worker to provide psychosocial support to patients and families across inpatient and ambulatory settings, including discharge planning and care coordination.

Qualifications

• Master's degree in Social Work required.
• Currently licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners required.
• For units/departments other than UAB Hospital, certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment.
• If job is in UAB Hospital, successful completion of First Five Rapid Response Basics online training required within 30 days of hire into position and must be maintained annually.

Responsibilities

• Provide individuals, families and groups with psychosocial support to cope with chronic, acute or terminal illnesses.
• Provide services including advising family caregivers, providing patient education and counseling, and making referrals for other services.
• May also provide care and case management or interventions designed to promote health, prevent disease and address barriers to access to healthcare.
• Facilitate the discharge planning process and identify community resources to assist patients and their families.
• Act as a clinical resource to both social service students and the community.
