Job summary

UAB Medicine in Birmingham, AL seeks a licensed social worker to support patients and families across inpatient and ambulatory settings. You will conduct psychosocial assessments, coordinate discharge planning, and connect patients with community resources.

Qualifications

• Master's degree in Social Work is required.
• Work experience may NOT substitute for education requirement.
• Currently licensed or eligible to be licensed as a Licensed Master Social Worker (LMSW) by the Alabama State Board of Social Work Examiners is required. If job is in TKC, license must be obtained within six months of employment.

Responsibilities

• Assist patients and families in accessing national, state, institutional, community, and pharmaceutical resources.
• Provide social evaluation and placement in medical records.
• Provide counseling to patients/families and/or groups.
• Participate in staffing, clinics, and rounds to support psychosocial understanding and care coordination.
• Facilitate discharge planning and arrange services such as home health, hospice, and SNF placement.
• Respond to patient population needs; may supervise students; act as community liaison.
