Job summary

North Alabama Medical Center in Florence, AL is seeking a Social Worker (LBSW) to join our case-management team. This full-time role focuses on helping patients cope with social, emotional, economic, and environmental challenges and coordinating services.

You will assess patients, participate in discharge planning, provide therapy, and collaborate with the health-care team. Qualifications include a Bachelor's degree, Alabama LBSW licensure within 6 months, and Handle With Care certification

Qualifications

• Bachelor's degree required.
• Alabama LBSW licensure within 6 months of hire.
• Handle With Care certification within 30 days of hire.
• Requires critical thinking skills, decisive judgment, and ability to work with minimal supervision.

Responsibilities

• Interviews and assesses patients and/or families and caregivers.
• Determines, prioritizes, provides and arranges for needed internal and external services.
• Participates in case reviews with the healthcare team to monitor progress.
• Assists with discharge planning and identifies resources for housing, transportation and healthcare services.
• Supervises and trains new staff, students and interns.
• Provides individual, family and group therapy.
