The Client Care Coordinator at Children's Rescue Fund coordinates mental health services for homeless families in New York City, delivering comprehensive bio-psychosocial assessments and linking families to community resources to support independent living.

The role requires an LMSW in NY, strong collaboration with a multidisciplinary team, crisis intervention, case management, and meticulous documentation. A sign-on bonus of 1,500 is offered; bilingual abilities are a plus.

Qualifications

• Must be LMSW in NY at hire or within 3 months and hold a MSW from an accredited school.
• Minimum of two years of experience in social services, counseling, crisis intervention, family or homeless services.
• Knowledge of TANF, Social Security, Medicare and other entitlements.
• Experience working with diverse cultures; strengths-based, family-centered practice.

Responsabilités

• Complete comprehensive bio-psychosocial assessments for families to assess needs.
• Assist homeless families navigating multiple systems and cope with stressors.
• Improve access to mental health services for families in shelter.
• Provide clear written reports with findings and recommendations.
• Work within a multidisciplinary team to enhance engagement and service quality.
• Coordinate safety planning and psycho-education for families.
• Refer and link families to housing, government agencies and community services.
• Document timely case notes and participate in family meetings.
