Children's Rescue Fund is seeking a Licensed Social Worker to serve as Client Care Coordinator for homeless families in New York City. This full-time role focuses on assessment, case coordination, and linkage to community services to promote family stability.

The ideal candidate will have LMSW with NY licensure, at least two years' experience, strong written and verbal communication, and the ability to work in a multidisciplinary team. A $1,500 sign-on bonus is offered for qualified hires.

Qualifikationen

• Must be LMSW in NY at hire or within 3 months.
• Master’s Degree in Social Work from an accredited program.
• Minimum of two years in social services, counseling, crisis intervention, housing or homeless services.

• Perform comprehensive bio-psychosocial assessments for families.
• Assist homeless families navigating multiple systems and stressors.
• Improve access to mental health services for families in shelter.
• Provide clear written assessment reports with recommendations.
• Collaborate with a multidisciplinary team to enhance engagement and service quality.
• Coordinate with agencies to link families to community resources.
• Conduct outreach, crisis intervention, safety planning and psycho-education.
