Sierra Vista Child & Family Services in Modesto, CA is seeking a Peer Navigator / Peer Support Specialist to join our Intensive Community Support program, bringing lived experience to support individuals and families and collaborating with the treatment team. This frontline role requires CA Peer Support certification, strong communication, and the ability to connect clients with community resources.

Competenze

• Must have high school diploma or GED.
• Lived experience in mental health.
• Current Peer Support Specialist certification; CA certifications must stay active.
• Proficient with Word, Excel and PowerPoint.
• Knowledge of community resources preferred.
• Bilingual preferred.
• CA driver’s license required for staff with driving duties.

Mansioni

• Provide the perspective of lived experience to clients.
• Assist clients with accessing and linking to community resources.
• Help clients and family members with problem solving.
• Provide encouragement and hope.
• Deliver group, individual, and family services for clients.
• Serve families, including non-traditional hours when needed.
• Maintain documentation and tracking to agency standards.
• Collaborate with treatment teams and partner agencies to coordinate care.
• Collect outcome data and conduct satisfaction surveys.
• Participate in community outreach and presentations.
• Meet program goals and contractual requirements.
• Participate in scheduled peer review.
• Maintain infection control and promote quality and safety.
• Use de-escalation techniques for crisis prevention.
• Administer C-SSRS to help prevent suicide.
